Qualifying Medical Conditions for Medical Cannabis:

In Utah, medical cannabis is available for patients with qualifying medical conditions. These conditions are diagnosed by a qualified medical provider and require a medical cannabis card from the Utah Department of Health. Listed below are all the current qualifying conditions in the state of Utah: 

  •  Epilepsy / debilitating seizures

  •  Multiple Sclerosis

  •  PTSD - With treatment records from a therapist

  •  Autism

  •  Terminal Illness

  •  Receiving hospice care

  •  Pain lasting longer than 2 weeks

  •  Rare condition(s)

  •  HIV or AIDS

  •  Alzheimer's Disease

  •  Amyotrophic Lateral Sclerosis

  •  Cancer

  •  Cachexia

  •  Persistent nausea

  •  Crohn's disease

  • Must be 21 years or older 

For Patients Under The Age of 21

If you're a minor, you may still qualify for a medical cannabis card if you have a documented disability or certain qualifying conditions. Please note, anyone under the age of 21 will have to go through the compassionate youth board when applying for a medical card. We encourage you to reach out to us and we can discuss this further.

  • Initial Consultations: $165

  • 12-Month Renewal: $150
