  •  Epilepsy / debilitating seizures

  •  Multiple Sclerosis

  •  PTSD - With treatment records from a therapist

  •  Autism

  •  Terminal Illness

  •  Receiving hospice care

  •  Pain lasting longer than 2 weeks

  •  Rare condition(s)

  •  HIV or AIDS

  •  Alzheimer's Disease

  •  Amyotrophic Lateral Sclerosis

  •  Cancer

  •  Cachexia

  •  Persistent nausea

  •  Crohn's disease

  • Must be 21 years or older

Register for your EVS.Utah.Gov account 

To access Utah's medical cannabis program, individuals are required to register on the state's EVS website at evs.utah.gov. This user-friendly platform serves as a pivotal hub for the registration process, facilitating the necessary steps to obtain a medical cannabis card

Book an appointment with our medical providers 

Once the registration process on the state's EVS website at evs.utah.gov is successfully completed, individuals can take the next step towards accessing medical cannabis by scheduling an appointment with one of our dedicated medical providers.
